1. An applicant for a license to engage in the business of a private professional guardian in this State must file with the Commissioner an application on a form prescribed by the Commissioner, which must contain or be accompanied by such information as is required. 2. A nonrefundable fee of not more than $750 must accompany the application. The applicant must also pay such reasonable additional expenses incurred in the process of investigation as the Commissioner deems necessary. 3. The application must contain:

(a)The name of the applicant and the name under which the applicant does business or expects to do business, if different.
(b)The complete business and residence addresses of the applicant.
(c)The character of the business sought to be carried on.
(d)The address of any locatio
