1. Upon determining that an employer is qualified as a self-insured employer, the Commissioner shall issue a certificate to that effect to the employer and the Administrator. No certificate may be issued to any employer who, within the 2 years immediately preceding the employer’s application, has had his or her certification as a self-insured employer involuntarily withdrawn by the Commissioner. 2. A certificate issued pursuant to this section must include, without limitation:
