Text
Every licensee shall give to every applicant for employment from whom a fee is required for securing employment an invoice on which is stated:
1.The name and address of the employment agency;
2.The name and address of the employer;
3.The name of the applicant;
4.The date;
5.The amount of the fee required for the services of the employment agency;
6.The title of the position secured by the applicant;
7.The name of the person authorized to hire employees for the employer; and
8.The amount of the gross cash wage.
