1. The application must include the following information in the following order:

(a)The name and address of the person guaranteeing the registration.
(b)The brand and grade.
(c)The guaranteed analysis showing the minimum percentage and source of plant food claimed in the following order and form: Total nitrogen: Percentage ........, source ........
