Each holder of a permit shall maintain accurate records upon such forms as may be provided by the health authority and containing such information as may be reasonably required by the board concerning the care or transportation of each patient, or both, within this state and beyond its limits. These records must be available for inspection by the health authority at any reasonable time and copies thereof must be furnished to the health authority upon request. This record does not constitute a diagnosis, and a legal signature is not required on forms dealing with the type of injury sustained by a particular patient. The health authority shall compile and provide a summary of this information.
