1. A denial of rights of a person with a disability who is a patient of a facility pursuant to NRS 449A.200 to 449A.263 , inclusive, must be entered in the patient’s record. Notice of the denial must be provided to the administrator of the facility. 2. If the administrator of a facility receives notice of a denial of rights pursuant to subsection 1, the administrator shall cause a full report to be prepared which must set forth in detail the factual circumstances surrounding the denial. A copy of the report must be provided to the Division. 3. The Division:
