Text
1. The Board shall prescribe, and the State Registrar shall furnish in sufficient numbers to each county clerk for distribution, a form for the reporting of divorces and annulments of marriage. 2. The information required by such form must be limited to:
(a)The names of the parties;
(b)The court and county in which the decree is granted; and
(c)The date of the decree.
