If the system for the reporting of information on neurodegenerative diseases is established pursuant to NRS 439.5268, the designated department, Chief Medical Officer and Department shall not reveal the identity of any patient, provider of health care or health care facility which is involved in the reporting of information to the system established pursuant to NRS 439.5268, unless the patient, provider of health care or health care facility gives prior written consent to such a disclosure.
