Text

1. A provider shall provide to potential clients a form for a written record of a physical examination. The form must:

(a)Describe with particularity the physical demands of the outdoor youth program and the environment where the program will be conducted; and
(b)Contain spaces for the results of:
(1)A test of urine for the presence of a controlled substance;
(2)An examination of blood count;
(3)A test of urine for infections;
(4)An examination of electrolytes;
(5)A test for pregnancy;
(6)An assessment of ability to cope with physical stress; and
(7)A determination by the physician as to whether detoxification is necessary before enrollment in the outdoor youth program. 2. A field administrator shall not allow a client to participate in an outdoor youth program unless the field ad
