“Information maintained by an agency which provides child welfare services” means data or information concerning reports and investigations made pursuant to NRS 392.275 to 392.365, inclusive, including, without limitation, the name, address, date of birth, social security number and the image or likeness of any child, family member of any child and reporting party or source, whether primary or collateral.
