A subpoena or subpoena duces tecum may be in substantially the following form: Subpoena The State of Nevada to A. B.: You are commanded to appear before C. D., a justice of the peace of ................ township, in ................ County (or, the court of ................, as the case may be), at (naming the place), on (stating the day and hour), as a witness in a criminal action, prosecuted by the State of Nevada against E.

F.Given under my hand this ........ day of the month of ............ of the year ....... .
