When a guardian has taken the official oath and filed a bond as provided in this chapter, the court shall order letters of guardianship to issue to the guardian. Letters of guardianship may be in the following form:

(name of guardian) ...........................(guardian of the person or estate or person and estate or special guardian) for ........................
