Any working interest owner may file an application with the division requesting an order for the unit operation of a pool or any part thereof. The application shall contain: A. a description of the proposed unit area and the vertical limits to be included therein with a map or plat thereof attached; B. a statement that the reservoir or portion thereof involved in the application has been reasonably defined by development; C. a statement of the type of operations contemplated for the unit area; D. a copy of a proposed plan of unitization which the applicant considers fair, reasonable and equitable; E. a copy of a proposed operating plan covering the manner in which the unit will be supervised and managed and costs allocated and paid; and F. an allegation of the facts required to be found by
