§ 26:4-133 — Definitions relative to Statewide automated and electronic immunization registry.
New Jersey·Title 26 HEALTH AND VITAL STATISTICS
3.As used in this act: "Commissioner" means the Commissioner of Health. "Department" means the Department of Health. "Health care provider" means a health care facility licensed pursuant to P.L.1971, c.136 (C.26:2H-1 et seq.) or a health care professional whose practice is regulated pursuant to Title 45 of the Revised Statutes. "Registry" means the New Jersey Immunization Information System established pursuant to this act. L.2004, c.138, s.3; amended 2012, c.17, s.339.
