§ 5-C:81 — Queries
New Hampshire·Title I THE STATE AND ITS GOVERNMENT·Ch. 5-C VITAL RECORDS ADMINISTRATION·Subdivision Death Registration Forms and Procedures
I.When the division receives the death record, it shall be examined for accuracy and completeness.
II.Queries to obtain missing information or to clarify submitted information shall be made by the division to the attending or certifying physician, APRN, or physician associate concerning the cause of death and related information, and to the funeral director, next of kin, or designated agent concerning all other information.
