Text
The burial permit for the disposition of fetal remains shall consist of the following information provided by the persons indicated. The hospital administrator or designee shall provide: the name, address, and license number, if any, of the funeral director, next of kin, or designated agent; the name of fetus; the date that the permit is completed; the signature of the hospital administrator or designee; and the name of the hospital. The cemetery or crematory authority shall provide: the manner of disposition; the date of disposition; the name of the cemetery or crematory; the name of the place where the cemetery or crematory is located; the section, lot number, and grave number; and, the signature of the funeral director, next of kin, or designated agent, the license number of the funeral
