§ 5-C:72 — Disinterment and Reinterment Forms

New Hampshire·Title I THE STATE AND ITS GOVERNMENT·Ch. 5-C VITAL RECORDS ADMINISTRATION·Subdivision Death Registration Forms and Procedures

I. A permit for disinterment, transit, and reinterment shall be obtained from the division prior to disinterment of a dead body, except as exempted by RSA 290:5. An application for disinterment and reinterment shall not be processed by the division unless it includes the following:

(a)The applicant's relationship to the deceased.
(b)The name of the deceased.
(c)The date, place, and cause of death.
(d)The name and location of the cemetery where the body is presently buried.
(e)The name and location of the cemetery for reinterment.
(f)The name and address of the funeral director.
(g)The applicant's signature and address.
(h)Identification of the applicant as either the next of kin or a person authorized by the next of kin to the deceased. II. The application shall become the permit f
