In this subdivision: I. "Disability" means a physical or mental impairment that substantially limits one or more major life activities. II. "Home and community-based services" means a range of medical and supportive services provided to persons in their own homes or other community-based settings including, but not limited to, adult day programs, and assisted living. III. "Long-term services and supports"means a variety of services provided in both facilities and community-based settings designed to meet a person's health or personal care needs to help them live as independently and safely as possible when they can no longer perform everyday activities on their own. IV. "Older adult" means an individual who is 60 years of age or older. V. "System of care" means:
