The certificate of acknowledgment by an attorney in fact must be substantially in the
following form:
STATE OF NORTH DAKOTA)
County of ___________ )
On this _____ day of _____________, in the year _________ before me (here insert
the name and quality of the officer), personally appeared ___________, known to me (or
proved to me on the oath of _________________) to be the person who is described in and
whose name is subscribed to the within instrument as the attorney in fact of
_____________ and acknowledged to me that that person subscribed the name of
_______________ thereto as principal and that person's own name as attorney in fact.
