A certificate of acknowledgment, unless otherwise provided in this chapter, must be in
substantially the following form:
STATE OF NORTH DAKOTA)
County of ___________ )
On this _____ day of __________, in the year _________ before me personally
appeared _____________, known to me (or proved to me on oath of ________) to be the
person who is described in and who executed the within instrument, and acknowledged to
me that that person (or they) executed the same.
