Each practicing physician in this state shall report to the local board of health within the
jurisdiction of which the death occurred, in writing, the death of any of the physician's patients
who has died of any contagious or infectious disease. The report must be made within
twenty-four hours after such death and must state the specific name and character of the
disease.
