and human services.
At such time as may be required by the department of health and human services, each
local health officer shall submit to such department, on blanks furnished by the department for
that purpose, a summarized report of the reportable diseases reported to the health officer
during the week. When no cases have been reported during the week, the report must be made
with the notation "No cases reported".
