Securities required to be registered by qualification under this chapter before they may be sold in this state must be registered as provided in this section. Application for registration of securities by qualification must be made by the issuer of the securities or by a registered broker-dealer by filing with the department: 1. An application for registration, which must be made in writing or on forms prescribed by the commissioner, must contain the following information and be accompanied by the following documents: a. With respect to the applicant or issuer and any significant subsidiary:

(1)Its name, address, and form of organization;
(2)The state of foreign jurisdiction and date of its organization;
(3)The general character and location of its business;
(4)A general description of
