50-32-403 . Common reporting form.

(1)The department of justice must provide a common reporting form for the report required under 50-32-401 .
(2)The form must contain the following information:
(a)name of the substance;
(b)quantity of the substance sold, transferred, or furnished;
(c)the date the substance was sold, transferred, or furnished;
(d)the name and address of the person buying or receiving the substance; and
(e)the name and address of the manufacturer, wholesaler, or retailer.
