35-14-121 . Forms.

(1)(a) The secretary of state may prescribe and furnish on request forms for:
(i)an application for a certificate of existence or certificate of registration;
(ii)a foreign corporation's registration statement;
(iii)a foreign corporation's statement of withdrawal;
(iv)a foreign corporation's transfer of registration statement; and
(v)the annual report.
(b)If the secretary of state requires, use of these forms is mandatory.
(2)The secretary of state may prescribe and furnish on request forms for other documents required or permitted to be filed by this chapter, but their use is not mandatory.
