§ 301.219 — Application to be submitted, contents — fee.

Application for a license shall be submitted biennially and shall be made on the form the department prescribes, containing the name of the applicant, the address where business is to be conducted, the kind of business, enumerated in section 301.218 to be conducted, the residence address of the applicant if an individual, the names and residence addresses of the partners of the applicant if a partnership, the names and residence addresses of the principal officers of the applicant and the state of its incorporation, if a corporation.  The application shall be verified by the oath or affirmation of the applicant, if the applicant is a partnership or a corporation, by a partner or officer of the applicant and shall be accompanied by a fee of one hundred thirty dollars every two years for each kind of business required to be licensed under subdivision (1), (2), (3), or (4) of subsection 1 of section 301.218 .  If the applicant conducts business at different locations, a separate application, license and one hundred thirty dollar fee shall be required for each location.  The director may stagger the expiration dates to equalize the workload.
