Any licensee allowing a license to lapse may within two years of the lapse request that their license be returned to active status by notifying the department in advance of such intention, and submit a complete application upon such forms as prescribed by the department in rules adopted pursuant to sections 190.001 to 190.245 .  If the licensee meets all the requirements for relicensure, the department shall issue a new emergency medical technician license to the licensee.
