Subdivision 1.Grounds for suspension or revocation. The commissioner may suspend or revoke the certificate of authority issued to a prepaid limited health service organization under sections62A.451to62A.4528upon determining that any of the following conditions exist:

(1)the prepaid limited health service organization is operating significantly in contravention of its basic organizational document or in a manner contrary to that described in and reasonably inferred from any other information submitted under section62A.4512, unless amendments to the submissions have been filed with and approved by the commissioner;
(2)the prepaid limited health service organization issues an evidence of coverage that does not comply with the requirements of section62A.4516;
