Subdivision 1.Application. The application for a license to operate and maintain a kennel or operate as a dealer shall be made to the Board of Animal Health, in the manner prescribed by rules of the board. Subd. 2.Contents. The application for a license shall be in writing and on a form as the board may by rule provide, and shall set forth:

(1)The full name and address of the applicant or applicants, or names and addresses of the partners if the applicant is a partnership, or the names and addresses of the officers if the applicant is a corporation, and the address of the corporation.
(2)The legal description or, in its place, the address and specific location of the site, lot, field, or tract of land upon which it is proposed to operate and maintain a kennel. Subd. 3.Fees; issuance o
