Subdivision 1.Declaration. Every application for public assistance under this chapter or chapters 142G, 256B, 256D, and 256L; child care programs under chapter 142E; and the Supplemental Nutrition Assistance Program (SNAP) benefits under chapter 393 shall be in writing or reduced to writing as prescribed by the state agency and shall contain the following declaration which shall be signed by the applicant: "I declare under the penalties of perjury that this application has been examined by me and to the best of my knowledge is a true and correct statement of every material point. I understand that a person convicted of perjury may be sentenced to imprisonment of not more than five years or to payment of a fine of not more than $10,000, or both." Subd. 2.Penalty.
