Subdivision 1.Establishment. The commissioner of human services must establish the projects for assistance in transition from homelessness program to prevent or end homelessness for people with serious mental illness, substance use disorder, or co-occurring substance use disorder and ensure the commissioner achieves the goals of the housing mission statement in section245.461, subdivision 4. Subd. 2.Eligible applicants. An applicant for a grant under this section must be a nonprofit organization, county, or other entity that provides services to help individuals transition from homelessness.
