Subdivision 1.Form; fee. An applicant for licensure as a physical therapist or physical therapist assistant shall submit a written application on forms provided by the board together with the appropriate fee in the amount set by the board. No portion of the fee is refundable. No applicant will be approved to sit for the national examination until the application is complete, as determined by the board. Subd. 2.Contents of application.

(a)The application must include the following information:
(1)evidence satisfactory to the board that the applicant has met the educational requirements of section148.721or148.722as demonstrated by a certified copy of a transcript;
(2)recommendations by two physical therapists registered or licensed to practice physical therapy in the United States or Ca
