(a)An applicant for licensure must comply with the application requirements in section148.6420. To qualify for licensure, an applicant must satisfy one of the requirements in sections148.6408to148.6415, or section148.645and not be subject to denial of licensure under section148.6448.
(b)A person who applies for temporary licensure must meet the requirements in section148.6418.
(c)A person who applies for licensure under section148.6408or148.6410more than two years after the person's initial NBCOT certification was issued and who has not practiced in any jurisdiction must submit:
(1)a completed and signed application for licensure on forms provided by the board that meet the requirements of section148.6420, subdivision 1, paragraph (a), clauses (1) and (2); and
(2)proof of a minimum of
