MOTOR VEHICLE SERVICE AND REPAIR ACT (EXCERPT) Act 300 of 1974 257.1319 Loss, destruction, or mutilation of registration, certificate, or trainee permit; replacement. Sec. 19. In the event of loss, destruction, or mutilation of a registration, certificate, or trainee permit, the person to which it was issued may obtain a replacement by furnishing satisfactory proof of the loss, destruction, or mutilation and paying the fee required under section 30. An application for a replacement registration, certificate, or trainee permit shall include all of the following information:

(a)The name and address of the applicant.
(b)An explanation of the loss, destruction, or mutilation of the original registration, certificate, or permit.
