Text
An application for a recycler license must contain the following information in such form as the Secretary of State may prescribe:
1.Identification.
The applicant's name, type of business organization and place of organization;
2.History.
The qualifications and business history of the applicant and any partner, officer or director;
3.Criminal and civil record.
Whether the applicant has been found guilty of any criminal offense within the past 5 years or has had a judgment of liability in a civil action involving fraud, misrepresentation or conversion. For a corporation or partnership, the application must provide the information required in this subsection for all directors, officers or partners;
4.Place of business.
A satisfactory report from a representative of the Secretary of State
