Text
Every provider shall provide to its subscribers within 120 days following the close of its first fiscal year of operation:
1.Statement of financial condition.
The most recent certified annual statement of financial condition, including a balance sheet and summary of receipts and disbursements, including notes of that statement;
2.Description of structure and operation.
A description of the organizational structure and operation of the provider, including the kind and extent of subscriber participation and a summary of any material changes since the issuance of the last report;
3.Description of services.
A description of services and information as to where and how to secure them; and
4.Method of subscriber complaints.
