Every physician attending a birth after which the mother and child are not admitted to a hospital for postnatal care immediately after birth shall, within ten days after such birth, file with the clerk of the city or town wherein such birth occurred a report on forms prepared and furnished by the commissioner of public health, stating the facts required to be shown on the record of such birth.Chapter 46: Section 3B.
