§2120.68. License issuance; application; fees; onsite inspections A. Each application for licensure as a PACE provider shall be submitted to the department on forms prescribed by the department and shall contain information as the department may require. If a PACE organization operates in more than one departmental region, the PACE organization shall obtain a separate PACE provider license for each region. At a minimum, the licensing application shall contain all of the following information:

(1)Name, address, and contact information of the person, partnership, corporation, unincorporated association, or other legal entity that is the applicant.
(2)Names and addresses of the applicant's officers, directors, managers, and administrators, if applicable.
(3)A line of credit from a federall
