§ 27:312 — Revocation or suspension of a license; civil penalty

§312. Revocation or suspension of a license; civil penalty A. The board or division, as may be applicable, shall initiate an administrative action and may revoke or suspend the license or permit of any person issued pursuant to the provisions of this Chapter for any of the following:

(1)The failure to meet the requirements of suitability as defined in this Chapter or in any rules adopted by the board.
(2)The failure to meet the requirements for the issuance of a license as provided for in this Chapter or in any rules adopted by the board.
(3)Repeated violations of any of the provisions of this Chapter or any rule of the board governing this Chapter. "Repeated violations" shall mean three violations of the same rule or statutory provision which have occurred on separate occasions by the
