§ 22:252 — Annual report

§252. Annual report
A.On or before the first day of March, every health maintenance organization shall
annually file a report with the commissioner covering the preceding calendar year and shall
file a copy of the report with the secretary of the Louisiana Department of Health. The report
shall be verified by at least two principal officers or duly authorized representatives of the
health maintenance organization. The report shall be on forms prescribed by the National
Association of Insurance Commissioners. The report shall be accompanied by the filing fee
and any penalty for unauthorized late filing set forth in R.S. 22:269.
B.The commissioner for good cause may grant an extension of up to thirty days for
the filing of the annual report.
