§1-121. Forms A.

(1)The secretary of state may prescribe and furnish on request forms for any of the following:
(a)An application for a certificate of existence and standing.
(b)A foreign corporation's application for a certificate of authority to do business in this state.
(c)A foreign corporation's application for a certificate of withdrawal.
(d)The annual report.
(2)If the secretary of state so requires, use of these forms is mandatory. B. The secretary of state may prescribe and furnish forms for other documents required or permitted to be filed by this Chapter.
