§ 40-1127 — Same; content of reports
Kansas·Ch. 40 INSURANCE·Art. 11 GENERAL PROVISIONS RELATIVE TO CASUALTY, SURETY AND FIDELITY COMPANIES
The reports required by subsection (a) of K.S.A. 40-1126 shall contain:
(a)The insured's name and license number issued by the state board of healing arts;
(b)date of occurrence which created the action; and (c) the name of the plaintiff and the injured party, if different.
