5.

(a)Except as provided in subsection (b), if a program participant provides a person with:
(1)written notice on the form prescribed under section 1 of this chapter:
(A)that the individual is a program participant; and
(B)of the requirements of this section; and
(2)the designated address; the person shall accept the designated address as the address of the program participant and may not require the program participant to provide an address in place of, or in addition to, the designated address.
(b)A person may require a program participant who complies with subsection (a) to provide an address in addition to the designated address as a condition of receiving a service or benefit that would be impossible to provide without knowledge of the program participant's physical location. Ho
