Text

"Identifying information", for purposes of IC 31-19-9-6 and IC 31-19-17 through IC 31-19-25.5, means:

(1)any name that a party to an adoption has used or is using;
(2)any address that a party to an adoption has used or is using;
(3)the original certificates of birth stored with the Indiana department of health with evidence of adoption under IC 31-19-13-2; and
(4)any other information, except the medical history, that may identify a person as a party to an adoption or as a birth parent, an adoptee, or an adoptive parent. [Pre-1997 Recodification Citations: 31-3-1-6(h); 31-3-4-5.5.]
