5. "Qualified medical practitioner", for purposes of IC 31-9-2 means the following:
(1)A physician licensed under IC 25-22.5.
(2)A physician assistant licensed under IC 25-27.5.
(3)A physical therapist licensed under IC 25-27.
(4)An advanced practice registered nurse licensed under IC 25-23.
(5)A chiropractor licensed under IC 25-10.
(6)A psychologist licensed under IC 25-33.
