Text

(a)The department shall approve a request made under this chapter if:
(1)the child lived with the individual making the request in the eighteen (18) months preceding the request; and
(2)the child lived with the individual making the request for:
(A)at least six (6) months in any one (1) calendar year; or
(B)more than one-half (1/2) of the days the child lived in the calendar year if the child was born or died in the calendar year.
(b)If the department approves a request, the department shall:
(1)provide the child's Social Security number to the individual; and
(2)notify the parents of the child that the child's Social Security number has been disclosed under this chapter if parental rights for the child have not been terminated.
