§ 31-28-1-3 — Health summary record; contents
Indiana·Art. 28 CHILD SERVICES: FOSTER CARE AND·Ch. 1 Health Summary Records of Children Receiving Foster
The local office of the county in which a foster child resides shall maintain a health summary record for the foster child. The provider that has provided ongoing care to the child shall complete the record. The record must include the following:
(1)A summary of health care provided to the child.
(2)Recommendations for future health care needs of the child.
