(a)When the licensed child placing agency or local office that will conduct the inspection and supervision required under IC 31-19-7-1 and submit the report required under IC 31-19-8-5 has been determined, the licensed child placing agency or local office shall provide to the child's current foster parent and to the prospective adoptive parent a document that contains the following information:
(1)A description of the information that must be provided to the court in the report required under IC 31-19-8-5.
(2)Contact information for individuals employed by the licensed child placing agency or local office in a supervisory capacity.
