Text
(a)Any person may voluntarily transmit
medical information to the state registrar for inclusion with the medical
history.
(b)The state registrar shall store all information received under this
section in a manner that makes the information readily recoverable.
(c)Any transmission of voluntary information must include an
affirmation by the person that:
(1)the information is true; or
(2)the person believes the information is true.
