§ 31-14-9-1 — Preparation of record of paternity determination
Upon a finding that a man is a child's biological father, the clerk of the court shall prepare a record of the paternity determination on a form prescribed and furnished by the Indiana department of health. The record must include the following:
(1)Facts necessary to locate and identify the birth certificate of
the child whose paternity has been established.
(2)A notice from the court indicating that the child's paternity has
been established in a court proceeding under this article (or IC 31-6-6.1 before its repeal), including identification of the court
action and proceedings.
(3)The name and address of the child's father.
